   • Chains / Cables (if fitted): Equaliser cables/chains in good condition, no fraying, wear, stretching, or damage.
   • Work Area Safety: Floor clean and clear, adequate lighting, no trip hazards, safe access around hoist.
   • Site-Specific Requirements: Safety decals and warning labels readable, tagging/current inspection status (if used), PPE confirmed.

3. Defect Reporting Section:

   • Space for operators to record any faults or issues.
   • Supervisor acknowledgment and actions taken.

4. Maintenance Log:

   • Record of completed repairs or servicing.

5. Operator Sign-Off:

   • Confirmation that the prestart has been completed.
   • Acknowledgment of any defects found.
